ECG of Last Week

This 65 year-old man had a history of recurrent syncope for which a single chamber ventricular pacemaker had been implanted at another center 3 years ago. During the last few months he presented with recurrent palpitations. This is a rhythm strip during one of the attacks.

Adenosine injection during the tachycardia revealed the following rhythm . What is the diagnosis?

This is another case of AV block needing pacemaker who presents with atrial tachycardia and rapid AV conduction. He refused RF ablation.
